New Patient Forms
It is important that we understand your health needs from the very beginning. Please complete our new patient forms before your first visit. Click on the 'New Patient Form' link to download the form to your computer. Print the form, complete it and bring the completed form with you to your first visit.
What to Expect
During your first visit to Marietta Smiles, we conduct a complete dental examination which includes intraoral and extraoral digital X-rays and photographs of your teeth. A doctor and hygienist team will discuss your concerns and goals for your smile as well as evaluate the condition of your teeth and gums to make recommendations to keep your teeth and smile healthy.
You will be greeted with a smile upon checking in. If you have not already printed and filled out your new patient paper work, you will be asked to do so, so please arrive 10 minutes early.
You will then be taken back to our comfortable Private Hygiene rooms to be evaluated by one of our wonderful hygienists and doctors. If additional treatment is recommended, our treatment coordinator will go over all procedures and fees with you.

Calm Room Requirment
If you require use of our calm room, please let us know prior to your appointment, so that we can have the room set up and ready for your arrival!
